When to use 360 review template?
Consider using a 360 review form when there is a requirement to gather feedback on an individual's performance from various perspectives, including peers, managers, and direct reports.
This type of review provides a comprehensive view of an employee's strengths and areas for improvement, which can be valuable for their professional development.
Create
Navigate to Forms >> Templates >> Create >> 360 degree review
Template detail
Below fields are available in template details -
Name - Template name
Description - Description of the template with additional details about the template.
Distribution mode - Based on selected modes, relevant templates will be listed when you select triggers in Form rule automation. Currently, 360 review forms are cannot be automated thus the setting is by default set to manual and cannot be changed.
Other settings - define whether you want the Feedback and issues tab to appear within the form response page for the reviewer’s reference. Having these tabs enabled will allow reviewers to have feedback and Jira issue information handy for the team member they are reviewing.
Add these template details and click on Next.
Add questions
Using Add a field and the Fields settings tab, add any type of question available in the form builder. Read Manage Templates | Create. Form creation process is the same for every template; the difference is only for form users. For example, in this form user type is reviewer, and the same questionnaire is available for all reviewers, unlike the self and manager review templates. Workflows
In 360 review there is only one default workflow available. Read more about 360-degree review template workflow here.
Distribute
As explained in Manage distributions. The process of distribution is the same for all types of templates. However, in the case of 360 review, there are two differences -
Type of reviewers - In 360 review, Team members can be reviewed by their peers, direct reports, and managers. There can be multiple reviewers for each type.
Anonymity settings - 360 review additional settings for anonymity, as explained below.
Cosindering multiple types of reviewer types in 360 review, UpRaise allows to import an Excel file that contains the list of reviewers. Once can easylity prepare the file and export directly instead of manually setting up users to distribute forms. Read Bulk importing users to a distribution via Excel upload.
Once the entire setup is done, one can save, schedule, or distribute the form. read Manage distributions | Distribute, Save as draft or Schedule distribution.
Manage anonymity
Anonymity - This setting lets you decide who should be able to correlate responses and users. If none of the options are checked, nobody can identify the respondents
Self assessment - Check if self-assessment is required. If self-assessment is checked, the same questionnaire is assigned to team member for self-assessment.
View responses
It does not an end here! Once the form is distributed, admin/HR (whoever distributes the form) would want to keep an eye on the form statuses. They may want to generate some reports by exporting forms or create user-wise report, all of this is possible on the view responses page.
As shown in the above image, there are three tabs on the view responses page.
Forms - Displays all the forms in the distribution
Reports - Generate report with the given distribution right away, instead to navigating to reports section
Details - All the distribution details.
Forms
As shown in the above image, Forms tab is all about the forms and their information and actions. From this tab, the top chart shows the statuses of the forms.
Filters to search for specific set of users.
Plus icon allows adding more user pairs to the ongoing distribution.
Actions (…) beside the plus sign have actions like Export to Excel, Start/stop accepting inputs, and extend due date. These options work at the distribution level. Below that, provided is the list of users added in the form, like the team member and their reviewers from left to right.
Every form is displayed with its current status, scores, assignee, etc.
Green pencil icon dictates that form is still accepting input. When form is closed, red pencil icon will be displayed and it won’t accept any input.
Actions (…) have options like export to Excel and start/stop accepting input. This will be available for each team member and will work at the user level only.
View option to open and view individual form.
Actions (…) beside each user row have the option to print the form.
Reports
This tab has a 360 report. Clicking on the report generates a report for the current distribution. This is a handy way to generate a report instead of navigating to Reports section in the app. Read Reports.
Details
This section is quite self-explanatory. This tab contains all the distribution details added while creating a distribution. A handy way to check the distribution setup.
