When to use self and manager review template?
Self and manager review template is typically used for quarterly, semi-annual, or annual reviews where team members are being reviewed by managers. Apart from this, it also serves some other use cases like one-on-one discussions and confidential reviews. For these use cases, self and manager review template has different workflows. This document talks about how to use and manage distributions of self and manager review.
Create
Navigate to Forms >> Templates >> Create >> Self and manager review
Workflows
In Self and manager review, there are three types of workflows -
Team member assessment followed by the reviewer - Used most widely for manager’s review after the team members' self-assessment.
One-on-one discussions - Used to conduct one-on-one discussions
Team member and reviewer in parallel - In the first workflow, both the team member and reviewer cannot add responses at the same time, but it is possible in this workflow. Template details
Below fields are available in template details -
Name - Template name
Description - Description of the template with additional details about the template.
Distribution mode - Based on selected modes, relevant templates will be listed when you select triggers in Form rule automation.
Other settings - define whether you want the Feedback and issues tab to appear within the form response page for the reviewer’s reference. Having these tabs enabled will allow reviewers to have feedback and Jira issue information handy for the team member they are reviewing.
Add these template details and click on Next.
Add questions
Add a relevant questions and sections in the template. The form creation process is the same for every template; the difference is only for form users. For example, in this form, user types are team member, reviewer, and HR user. You can add separate questions for each type of reviewer, unlike 360 review templates, where the same questionnaire is available for all types of reviewers. Also, read more about questions and sections in the template and save the template. You can save it and distribute it later, or distribute it right away.
Distribute
As explained in Manage distributions. The process of distribution is the same for all types of templates. However, in the case of self and manager review, there are two differences -
Type of reviewers - Apart from the team member, it also includes HR users as an optional type of reviewer.
Import OKRs - Self and manager review additionally has an Import OKRs option that allows making OKRs a part of the review process.
Cosindering multiple types of reviewer types in self and manager review, UpRaise allows to import an Excel file that contains the list of reviewers. Once the entire set up is done, one can save, schedule, or distribute the form. read Manage distributions | Distribute, Save as draft or Schedule distribution.

View responses
After distribution, the admin/HR (whoever distributes the form) would want to keep an eye on the form statuses. They may want to generate some reports by exporting forms or creating user-wise reports, all of this is possible on the view responses page.

As shown in the above image, there are three tabs on the View Responses page.
Forms - Displays all the forms in the distribution
Reports - Generate a report with the given distribution right away, instead of navigating to the reports section
Details - All the distribution details.
Forms
As shown in the above image, the Forms tab is all about the forms and their information and actions. From this tab, the top chart shows the statuses of the forms.
Filters to search for a specific set of users.
Plus icon allows adding more user pairs to the ongoing distribution.
Actions (…) beside the plus sign have actions like Export to Excel, Start/stop accepting inputs, and Extend due date. These options work at the distribution level. Below that, provided is the list of users added in the form, like the team member, reviewers and HR users from left to right.
Every form is displayed with its current status, scores, assignee, etc.
Green pencil icon indicates that the form is still accepting input. When the form is closed, red pencil icon will be displayed and it won’t accept any input.
Actions (…) have options like Export to Excel, Start/stop accepting input, Print & Delete. This will be available for each team member and will work at the user level only.
View option to open and view the individual form.
Reports
This tab has 2 reports for self and manager reviews. Clicking on the report generates a report for the current distribution. This section is quite self-explanatory. This tab contains all the distribution details added while creating a distribution. A handy way to check the distribution setup.
